Initial Paperwork

This is a form to tell us about you, your goals, and any limitations you may have. This is also where you will submit your pictures so we can best see how to serve you! This form must be filled out prior to any program being designed.

Do the following steps.

1. Complete the form below (it may say "Fill Out Form" depending on your browser. If so, just click "Fill Out Form" and it will open the form).

2. Complete/Sign the contract when you get your login access to My PT Hub.

NOTE: We cannot (and will not) submit any programs to you without the above listed steps being completed.
